Detailed Transition Opportunities
The Possibilities Transition 18+ Adult Program provides opportunities for young adult students, ages 18 to 21, to increase their transition skills through real life opportunities across settings in the local community, Possibilities program and home. Student’s IEP team must determine that the student meets the required needs to enroll in this program prior to high school graduation, then on an annual basis in order for the student to continue enrollment. If the student is found eligible to participate in this program, the student is allowed to walk with their class at graduation, and will receive their official high school diploma upon completion of the program.
The Possibilities Transition 18+ Adult student will be provided an opportunity to learn and implement real life transition skills in the areas of Employment, Post-Secondary Education and Training, and Daily Living as they transition to become an independent adult.
The following is a summary of transition opportunities that are dependent upon Individual young adult transition needs identified by their IEP (Individualized Educational Plan) team, and also by availability of opportunities. The Possibilities Transition 18+ Adult program will partner with young adult students, their parents, school district service providers, local outside agencies, and local communities to provide various transition opportunities at school, home and in the real world. - Employment -Students will have the opportunity to increase their employment skills through participating in various opportunities such as internships, volunteering, Manufacturing Possibilities, taking behind the scene tours at local businesses, and work experience.
- Post-Secondary Education and Training – Students will have the opportunity to increase their Post-Secondary Education and Training skills through participating in various opportunities such as functional academics within the program, sessions with outside agencies such as MN Vocational Rehabilitation Services, and also attending classes through SouthWest Metro Educational Cooperative in the areas of career and technical training, and Practical Assessment Exploration System training (PAES Lab).
- Daily Living – Students will have the opportunity to increase their Daily Living skills through participating in various opportunities in the areas of Community, Recreation and Leisure, and Home Living.
